 Inserting multiple record in a table via checkbox

Hi,
I need to keep track of external exams doctors require for patients.
they select the patient and insert the exam they need to do.
I can get the patient information from our patient management program, butneed to keep track of exams we request.
The interface they want is a table of exams they can select
o exam1 o examn
o exam2 o examn+1
o exam3
etc etc (they want it this way because it is the copy of the paper version they have)
they can select as many exams they need
Is there a quick way to insert multiple records in a table in the above scenario?

Can I use an editable grid modifing the exam_id in a checkbox and insert all the exams cehcked in one go?

It is a simple tool for internal use only they still have to fill the paper form as well so we need to keep it quite simple

I have a table of exams with exam_id, description and price,
a table with request_id, patient_id, date and doctor
and a table that links the two above tables request_id and exam_id this one is the one I need to populate.
For example if for request one exam 1 and 4 are selected I need to insert 2 records 1,1 and 1,4

Hope the scenario is clear enough (English is not my language)

Have you looked at the provided CCS examples?? There may be something there that can assist you in solving this problem or have you already solved this problem??

I didn't use codecharge for this
will use only the reporting from ccs
