I'm using HTML2PDF to create PDF files. My current struggles...

1. How to setup a header and/or a footer for each page.

My attempts at setting up a header have failed completely.

My attempts at setting up a footer worked only to the extent that I get
the footer on each page with no change to the page numbers.

2. Is there a good source for examples and/or documentation?

I've searched, wandered, read thru code (html2pdf & fpdf). I found
a few examples, but not very many.

Thanks in advance for any hints/help offered.
