 Duplicate Inserts when form is part of include page

I have 3 pages...
- one is the main presentation page and has 2 included pages:
- summary grid of another table
- report and a record to add rows to the same table (billable hours), there is no custom insert code in the record form

in this format the record form creates 2 new records on insert
  
Main Page  
- include page 1  
- include page 2  
   - report grid  
   - panel 1 starts  
      - record form  
   - panel 1 ends  

in this layout with the record cut & pasted into the main page, the operation works properly - in both cases i have the record return page set to the main page
  
Main Page  
- include page 1  
- include page 2  
   - report grid  
- record form on main page now  

Any suggestions or similar experiences would be appreciated - i think i can work around the inconvenience, but was trying to separate the parts of the page into chunks for easier editing.
