 Flash Video Embedded within a Page

I do volunteer work with the National Disaster Search Dog Foundation and we video tape dog which have to go through a 10 test and be rated. I would be usefull to have the video (in Flash format) embedded next to the test performed so the foundation can determine if the dog meets the requirements.

Does any know how best to approach embedding a flash video into the page? I plan on using the multiple file upload feature to get the video up to the server and would like to associate each video with a specific test.

Initial thought

Test1 --> Video Test 1 -- Upload button
Test2 --> Video Test 2 -- Upload button
etc....

The best way to do this would be to have a standard naming convention like, test1.flv -- test2.flv, test3.flv etc....

Then when you do a multiple file upload it would assign the video to the right test.

Any guidance would be much appreciated

Yes, the only difference is that the video will already be in FLV so there is no conversion. All I need to do is upload the video and have it presented in the form. Any help would be truly appreciated.
