 How do you Model, Control and Document your entire application generated with CCS?

Hello,

I would like to improve and standardize my working model of web application development,
I would like to make per project:

1.- A Visual Model: a well conceptualized graphical model with all the tasks that the application should do in a graphical and orderly way,

2.- Development and Version Control: Control all the changes of a CCS4 project and keep track of their files and changes, including the modeling files, ccp, php, sql, js, css, xHTML, psd, doc, fla, swf, documentation, etc. of all the project,

3.- Documentation: To carry the documentation in parallel with these 2 steps above.

Can these 3 steps be done with a single tool, besides CCS4, that do not cost thousands of dollars, or better, if it is good and open source? :-)

I appreciate a lot your recommendations, if possible please include links.
