I have a page containing about a 120 downloadable files (links)

I need to capture basic data from users downloading - name, email addreess
and file being accessed
(I appreciate there is no way I can varify the data entered)

I thought a simple form but without resorting a form for each item ....
How can create a generic form and pass the details of the link being
accessed to a form??

Select file page - go to download page with form to capture data - once complete file downloads?
