Hi,
I would like to build some simple PHP page in CCS 3.0 with survey grid. There are 30 questions, each question has 4 answers (A, B, C, D). User should choose only the one answer in question. Page should save answer for each question to SQL table with info about user, date and time.
Do you have any idea how to create it in simple way?

use radio button to select only one answer
use hidden fields with default values of CCGetUserID(), CurrentDate
