Is there any survey application out there that any one knows, one that incorporate different types of questions and can score as well!

what do you mean by score, do you mean a page full of questions similar to the website Poll on the forum page, with results pages full of all questions results
or do you mean a question/ multi choice system that gives correct answers out of 100 = xx
?
ken Hardwick has quiz stuff downloadable on his website , maybe thats what you mean?
