 Need advice on how to handle a Form / email etc

Hi,

Trying to see if this is possible in CodeCharge (not Studio), but have no idea on how to setup the Database to handle it or the forms/pages in CC (can do everything else, but for the life of me, this is confusing me).

We have 10 employees, and 4 supervisors.


We need to have a form accessible via web, where employees can send a specific form (absentee/vacaion request) form to their supervisor (I would like it to be a link/url to a page); then that supervisor puts their "approval"/"deny" on it, then sends it to their boss; that boss needs to put their "approval"/"deny" on it. When it reaches the boss, once the boss puts their "approval"/"deny" on it; the form, an email will be sent to the employee who put in that request, with a url to that online version of the form as well as a copy of it in their email.


So structure:

Absentee Form > Employee Fills out > Sent to their supervisor - in form of Email and URL to page with details of form (employee also gets a copy via email)> Supervisor "signs" the page (approve/deny) and on submit, goes to their boss in the form of an email, and the page gets updated to reflect the supervisors answer > Boss signs the page ( approve deny) and on submit, email is sent to Supervisor and Employee with a copy of the form in the email and also a link to the page.

Any ideas on how to set up at least the deny/approval part, and that ONLY the supervisor/boss its sent to is allowed to sign it...etc.

Something like the support form on Codecharge's website, but for CodeCharge (instead of Studio) is probably close to what I am looking for. Is the code for that available in CC format?
