Undefined form when trying to use datepicker

Hi, I am trying to implement the datepicker as shown on http://www.gotocode.com/art.asp?art_id=57& but am running into problems. In the caption section i have renamed my form and field accordingly but when I run it in explorer I get an error message saying that the form is undefined? Can neone help or point me in the direction to an example, btw Im using CC2 if it makes a difference. Cheers
