This has got to be a rookie mistake.
I let Application builder generate some pages. I can add my own html around the generated code until I try to enclose the generated code in a TABLE. Then the FORM and the rest of the output from the generated code disappears. It's there in the "Design" and "Preview" windows and when I access "Example.html" on the WEB live, but disappears from the results of "Example.php".

App. Builder gave me a CSS that I didn't want.
