For some unknown reason CCS beta 4 has been removing <form> tags, not always
but it appears to have something to do with includes, maybe. If your working
away and a page stops working (submit/update/cancel) check for <form> tags.
There should be a <form> tag on the line after <!-- BEGIN Record and one
</form> tag one line before <!--END
