 Odd entry problem

:(:(I have a simple ad/delete grid to add records to a MySql db using PHP and CCS. I have the max number of rows shown on the page set to 12. I can make 2 entries without a problem, the third entry sets itself and the previous record the same as itself. In other words the 1st record is untouched and the second and third are the same. The db is actually getting two distince records entered, I checked with phpMyAdmin.

Has anyone seen this?? I need to get this rectified right away, I need to bill customers.:-D
