Howdy folks... I've been poking around the documentation for this, and did a cursory search through the forums, and can't find what I'm looking for, so hopefully, you will be patient with me if this does exist somewhere I haven't looked...
I'm putting in some custom SQL, and playing around with the SQL Parameter window. Under Types, I notice an entry for Form. What is this, and how is it used? Is this to draw information from another form on the page? If so, I can't figure out for the life of me, how to refer to an element in a given form.
If someone could help out with this, it would be appreciated.

The "Form" parameter type is used to obtain values of POST parameters (versus GET/URL). These are usually parameters submitted via a form. These are usually equal to the control's name on the form, without any prefix. For example "user_name". You can run the page, view the HTML source in the browser, then search for "name" to find the names of each parameter that is being posted.
