 how do I use dependent listboxes for record insert / update?

I have been round and round with the dependent listboxes sample (http://www.gotocode.com/art.asp?art_id=45) trying to understand how and where it's suppose to be implemented. Trying to use it for inserting / updating records. The sample works on my setup, just how is it suppose to be used?
PHP4-Templates (for the sample), MySql, CC V2.0.5. Any way to use it without templates?
Any comments will be greatly appreciated.

Rob,
Dependent listboxes example you mentioned was created specially for template version (as you see template variables are used to build dependent functionality), there’s no templateless version as I know
