 Answering questions with checkbox options

Hello,
I need help regarding checkboxes. I have a survey form which requires users to select only 1 answer(checkbox) from options given. I really have no idea how to start with the form. I already went through to multiplecheckbox in the CCSExamplePack, but it is quite the opposite of the solution that i need. :(

Sounds like you are wanting to use radio buttons. See a sample of this at http://donsafar.no-ip.org/rbtest/ . Let me know if this is similar to what you are trying to do.

Thanks for your reply. It is somewhat similar..only 1 checkbox is allowed, but how about if there is multiple questions, with same answers(yes/no)/different set of answers for every question?
What i am trying to do is create questionaire forms that requires only 1 answer for every question from the users. How many tables(MySQL) do i need? Please advice more about this.
