 Passing Form parameters in URL

To add print function in my project we currently use Crystal. It spawns
in explorer due to the .rpt extension.., no problem. My question is how
do I pass parameters from codecharge forms to the url as in the example
below.

http://cbs2004.ctdhe.org/cbs/B100.rpt?init=actx&prompt0...pt1=B&prompt2=0

In this example I need to pass three paramters called prompt 0, 1 & 2.
Any/all suggestions would be greatly appreciated.
