I have to develop web application for change request forms approval. In this application user to attach word document
while inserting record. this document should be linked to database record.
Anybody knows about similar application.
Please provide me the link for the same.

As per your request i suggest you to use Persist upload method. In Code Charge knowledge base there is Sample Example for Upload you refer that and develope ur Application.
