addition of unique record based on multiple fields

i have a table in which the record has to be unique depening upon two or more fields.

for example

first name last name age
robert iny male 35
robert moonwalker male 22
julie iny female 26
julie gomez female 24

in the above example the record is unique only if all put togather are unique.

how to handle this in ccs.
