I am trying to create a survey page.

I have a list of 10 questions.

I want to display each question and a spot to fill in the answer in the same grid.

Such as:
1. How was the Course ? o good o better o best - (these are radio buttons)

But the questions of course will be in one database and the answers in another as the survey will be done by muliple people.
