 Multiple Headers

If you have a menu header that you want to stay on every page and then you have a form you want to go on the top of about 10-15 pages is there a quick way other than generating the top form each time to connect the pages with a different bottom?

Put this form to Header page and hide it based on some condition
(e.g. specifix page names) , see
