Also, does it require Apache (and what version) if not running IIS etc ?

CCS2 and CCS3 work with basically any SQL database, including MySQL 5.
CCS doesn't require any Web server. It is the Web application that needs to be placed on a Web server, therefore you'd need to use Web server that supports the technology that you have selected. If you selected ASP or ASP.NET then possibly only IIS can run such applications, but there may be alternatives as well. If you selected PHP then most Web servers should support it. Check the documentation of the Web server or the programming language to see if they are compatible.

We have APP runing in 5.0.18 not problem so far CCS3
