I would like to know how to use the checkbox option. I simply not able to use that. When I try to view it, it only appears one check box only.
For radio button it is perfectly fine.
Can someone help?

Winsland,
one checkbox field is created for one table field. You may list fields you want to be displayed and add checkbox field next to each field value. Look for related article on this site.
