 Dates being mangled?

I'm stumped. I have a task manager application similar to the tutorial, but I have a number of date fields.

For some reason, the date fields in the grid forms are blank, and when I update via a record form the DB records dates as strings of zeros (0000-00-00), rather than the date (2008-04-25)?

Any idea what I've done wrong and why the dates aren't being handled properly?

Standard Trobleshooting:

Be sure your date format is set (Settings) to yyyy-mm-dd

All date fields set to date with yyyy-mm-dd as the dbformat.
